The Case Manager coordinates the care and services across the continuum of illness, promotes the effective utilization and monitoring of healthcare resources, and assumes a leadership role with the interdisciplinary team to achieve optimal clinical outcomes and resource management.
Identifies discharge needs, coordinates planning and problem solves barriers to continuity of care.
QualificationsEducation:
Associates degree in healthcare related field requiredBachelors degrees preferred
Experience:
Minimum 5 years experience as a registered nurseExperience in case management or utilization review preferredLicensure/Certification/Registration:
Registered nurse in Virginia requiredCase Management Certification PreferredAdditional Skills/Requirements required:
Strong problem solving skillsAbility to apply critical thinking in a variety of situationsEffective verbal and written communication skills with all levels of staffBasic computer competencyAdvanced assessment and teaching skillsStrong Customer Relation skillsExcellent organizational skills with the ability to manage, prioritize, and follow through multiple tasks.
Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
